    Your medical history in one place.

    Through our online portal, patients may access their medical information electronically right away. You may obtain your information through a CDA (Clinical Data Architecture) report; you will receive two of these per visit: the first when you are discharged and the second when your codification is generated (this will take several days).

    • Discharge instructions

    Upon accessing our Patient’s Portal, you are accepting the following
    terms and conditions:

    1. I release Hospital Auxilio Mutuo from any legal responsibility that may arise from divulging this information and I renounce confidentiality rights regarding the documents provided through this portal.
    2. If the patient is not the person accessing this information, written consent signed by the patient must be provided. If it is not, the request to access the patient’s information will be declined.
    3. If the patient is a minor, a parent or legal guardian must come by the Departamento de Manejo de Información (Information Management Department) with a written and signed consent form. In the absence of said consent form, an original copy of the court order must be provided for the information request to be accepted.
    4. To confirm your information, you must type in your birth year (yyyy) in the invitation code field.
    5. Upon accepting the terms and conditions, the system will sync your medical information and redirect you to the Portal where you will be provided with your information.
