Our Health Information Management Department (“Departamento de Administración de Información de Salud”) carries out functions related to managing and keeping medical records through processes such as analysis, coding, filing, and information disclosure, manually and digitally.

Also, we offer services to facilitate your access to copies of your medical records; all you must do is complete the request form provided for this purpose at our department. Please remember to bring a photo ID; should the person requesting records not be the patient to whom these belong, a written and signed letter of authorization is required, as well as photo ID. The patient or the patient’s designee may access said request form via this page and bring it along with the documentation previously mentioned.
