The Discharge Utilization and Planning Department includes nursing professionals, medical advisors, clerical staff, and administrative personnel.
In this Department, case managers coordinate comprehensive and multidisciplinary discharge plans that cover all aspects of continuity of care, from the moment of the patient’s admission. Inpatients that require continued care in their homes may benefit from these resources.
All at-home services require a medical order and justification. Some health insurance plans require referrals made by the patient’s primary physician. These are coordinated using the patient’s insurance plan; authorization is granted for necessary services covered by the insurance plan’s provider network.
